Hmmmm I don't agree with Doyist, the monsters are shown in the very first chapter as "enemies of humanity" and Nana believed that the talented are these enemies. I think it was "foreshadowing".

Also with Watsonian, Nana's given a phone with a kill count to motivate her, every time she doubts she's reminded of the lives she's "saving". Indoctrinating a traumatized child would be easy, but it would be easier to carry out the masterplans etc. by showing her the monster before sending her to the island rather than the phone, then she wouldn't question how/why the talented are a threat. So maybe you're right and it was a recent acquire and is connected to Tsurouka's plans for Nakajima.
Keeping Rin alive is interesting too, he's been trying to recruit her and is testing cures on her instead of the hundreds of talented trapped in the confine.

Plus Nakajima's talent is nullifying talents so he's maybe he's trying to bass boost the hell out of his talent and use his brain cells as a cure or something and the "monsterfication" is a side affect?
There have been talents that have been very similar, so perhaps there was someone who had a similar talent or a type of nullification and that's who that basement monster is?

But what I wanna know is, why has Tsuruoka only shown the monster to Nakajima? Was it not around when Nana was indoctrinated? Or was it only to make Nakajima believe so hard that he really did transform?
