It feels a bit nonsensical to leave them sick. First of all they might already have developed some resistance. Second, why not cure them now? If they get sick later he can just keep detoxifying them, they'll slowly develop immunity never really being sick for long. Third, if it's an epidemic and they're leaving the country, the epidemic might be over when they're back anyways.

His sister is bringing attention to him, but I think he should first learn to say no, he's pretty much never rejecting anything.

It feels like a plot convenience, where the author just summons him to change something on the MC.
