The fights are mostly scripted, but the hits are real. If they just went all out it would just be UFC and injuries would be unsustainable. So wrestling has moves that are modified to either be less damaging than a normal move, or to give the receiver the chance to take them in a way that minimizes damage. They still need to sell that everything is real so the moves need to connect.

All that said, that shit still hurts and if performed poorly can still badly injure someone, so it takes practice and talent to be able to hit and be hit in a 'safe' way for as long as wrestlers do.
